Ministry Structure Inspection Report (MSIR)

Healthy ministry requires more than vision and good intentions. What we build must have the structure, leadership, systems, and capacity necessary to support the work God has entrusted to us.

The Ministry Structure Inspection Report (MSIR) is a practical companion resource to Built to FORM that helps ministry leaders move beneath visible activity and examine what is actually happening within the ministry’s structure.

The MSIR begins with the FORM findings from the book and helps leaders identify what is sound, what is under pressure, and what requires reinforcement before greater weight is added.

What This Resource Will Help You Do

Use the MSIR to:

  • examine the evidence behind your FORM findings;

  • identify structural strengths and recurring weaknesses;

  • evaluate leadership, order, systems, and ministry capacity;

  • recognize what pressure is revealing;

  • identify significant risks; and

  • establish reinforcement priorities and next steps.

The goal is not another score.

The goal is clarity that leads to responsible reinforcement.

Choose How You Want to Complete the MSIR

You can complete the Ministry Structure Inspection Report in either digital or printable format.

Complete the Digital MSIR

Use the interactive version to work through each area of the inspection, document what you are observing, identify reinforcement priorities, and develop your 90-Day Reinforcement Plan.

The digital MSIR guides you through the inspection one section at a time and brings your findings together into a completed Builder’s Inspection Report.

Complete the tool thoughtfully and based on evidence rather than assumption.

Your responses are not stored by the tool. Save or print your completed report before leaving if you want to return to your work later.

Download the Printable MSIR

Prefer to work on paper, complete the inspection during a leadership retreat, or work through it with a ministry team?

Download the printable Ministry Structure Inspection Report and use it alongside your copy of Built to FORM.

Use either version of the MSIR to move from ministry-health findings to deeper inspection, reinforcement, responsibility, and reinspection..

Begin With Your Built to FORM Baseline

Before beginning the MSIR, review your FORM Inspection and relevant chapter assessments in Built to FORM. Those findings provide the starting point for this resource.

You may have discovered that Formation is strong while Order needs attention. Reproduction may be inconsistent. Mission may be clear, but ministry systems may not yet have the capacity to support it. The MSIR helps you go beneath those findings and ask a deeper question: What is actually happening beneath the surface?

Rather than assigning another numerical score, the report uses four qualitative condition designations:

  • Sound: Functioning consistently and supporting healthy ministry

  • Needs Reinforcement: Generally functioning, but inconsistency or strain is evident

  • Significant Concern: Weakness is affecting people, stewardship, or ministry effectiveness

  • Immediate Attention: The condition presents serious spiritual, relational, operational, safety, financial, or leadership risk

These designations help leaders determine the level of attention a condition requires without turning the inspection into another scoring exercise.

What You Will Inspect

The MSIR guides leaders through several interconnected areas of ministry health.

Foundation & Mission examines whether Christ and Scripture remain the governing foundation of the ministry and whether actual practice remains accountable to the mission entrusted to the Church.

Formation focuses on the spiritual health, character, accountability, and leadership patterns shaping the ministry experience.

Order examines roles, authority, decision-making, communication, policies, accountability, and responsibility.

Reproduction considers how the ministry identifies, develops, supports, and releases people into responsibility.

Systems & Ministry Capacity looks at the repeatable systems that help people carry ministry responsibility consistently, including scheduling, follow-up, communication, financial stewardship, safety, technology, and continuity.

Load-Bearing Structures helps leaders identify the areas carrying the greatest structural weight in the ministry, including discipleship, leadership development, pastoral care, accountability, worship, prayer, teaching, and stewardship.

Pressure examines what becomes strained during growth, transition, conflict, financial pressure, volunteer shortages, crisis, or new opportunity.

Risk helps leaders identify areas that may require immediate attention or outside expertise, including leadership dependence, financial controls, safeguarding, technology, legal or regulatory compliance, and operational safety.

From Inspection to Reinforcement

Inspection should lead somewhere. After working through the ministry, the MSIR helps leaders identify repeated patterns, determine what should be preserved, reinforced, clarified, redesigned, or addressed immediately, and establish a focused 90-Day Reinforcement Plan.

The goal is not to create an endless list of improvements. It is to identify what matters most, assign responsibility, and take responsible action.

The report also includes a Builder’s Inspection Report and a reinspection process so leaders can return after changes have been implemented and determine what improved, what remains vulnerable, and what the next faithful step should be.

How to Use the Report

Work through the MSIR thoughtfully rather than trying to complete it quickly. Answer based on how the ministry actually functions, not simply how policies, organizational charts, or leadership expectations say it should function. The current article already emphasizes this practical approach, and that remains consistent with the finished resource.

For a broader ministry inspection, consider completing the report with a small leadership team. Bring your copy of Built to FORM, your FORM Inspection findings, ministry plans, organizational charts, policies, calendars, leadership lists, and relevant reports.

Most importantly, resist the urge to solve every issue while you are still identifying it.

Inspect first. Identify the pattern. Then determine the appropriate reinforcement.

A Biblical Foundation for Order

“Let all things be done decently and in order.”
1 Corinthians 14:40 NKJV

Biblical order is not bureaucracy for its own sake. Healthy order creates clarity, establishes appropriate responsibility, supports accountability, and helps ministry function faithfully.

The Ministry Structure Inspection Report helps us move beyond asking whether ministry activity is occurring. It asks whether the structure supporting that activity is healthy enough to carry the responsibility placed upon it.

Healthy builders inspect before they expand. Growth can reveal gaps. Pressure can reveal patterns. Reinforcement is wisdom, not failure. Faithful stewardship includes returning to inspect what we have built.
